The Final Conference of the SUNFRAIL project will be celebrated in Bologna, Italy on 7 February 2018. SUNFRAIL is co-funded under the Health Programme (2014-2020) and brings together 11 partners from 6 EU Member States. It is coordinated by Emilia-Romagna Region (ASSR and affiliated partner, Aster).
SUNFRAIL (Reference Sites Network for Prevention and Care of Frailty and Chronic Conditions in Community Dwelling Persons of EU Countries) aims at improving the identification, prevention and management of frailty and care of multimorbidity in community-dwelling persons (over 65) of loco-regional settings of EU countries.
SUNFRAIL Final Conference is dedicated to present the main results obtained during the 34 months of the SUNFRAIL project. It aims to propose recommendations for the replicability and sustainability of the model and of the tools identified. Furthermore, the conference focuses on programmes and initiatives in the field of frailty and multimorbidity. The SUNFRAIL Tool and model will be discussed as well as the opportunities that the project have resulted in.
The Final Conference will include panel and plenary discussions with high-level European and international speakers from the European Commission and national and regional health departments.
