SUMMARY:CoR Interregional Group on Health and Wellbeing - Meeting on "Integrated Care in Europe: the way ahead"
DESCRIPTION:The next meeting of the European Committee of the Regions’ Interregional Group on Health and Well-being will take place on Thursday 6 December at 8h00-9h00 at the European Committee of the Regions building\, room JDE 60. The topic of the meeting is \n“Integrated Care in Europe: the way ahead”\n  \nIn recent years\, Europe has faced important demographic changes affecting the ageing population. As a result\, increased chronic conditions\, multi-morbidity and an ageing population of both the workforce and patients have led to the development of a new integrated care model across Europe. Being able to successfully implement this model is key\, as the rising burden of chronic diseases and of the number of people with complex care needs now requires delivery systems that bring together a range of professionals and skills from both the cure (healthcare) and care (long-term and social care) sectors. \nAgainst this backdrop\, the next meeting of the European Committee of the Regions’ Interregional Group on Health and Well-being aims to discuss the challenges and the progress made in implementing integrated care models across the EU and give the floor to regional experiences and viewpoints on the matter. \nThe event will involve a presentation by a representative of the European Commission on the European Commission’s support for scaling-up of integrated care initiatives\, and a presentation by Isabella Notarangelo\, health economist at the European Hospital and Healthcare Federation (HOPE)\, who will give an overview on the ICT4Life Horizon 2020-funded project aimed at advancing Europe’s leadership role in personalised services for integrated care. \nThe meeting will conclude with a contribution by Professor Jonas Christensen from the Malmö University (SE) on AppSam – Professional Support in Dementia Care – a transnational project in the CareSam cross-border research network aimed at implementing a social innovation model.
